Mobile Pulmonary Navigator

Our Mobile Pulmonary Navigator is a registered respiratory therapist that will meet with you - in your home or virtually - and provide the necessary education, support, and assistance to help you manage your lung disease and navigate the complex health care system. The Pulmonary Navigator will identify any social determinants of health, as well as any physical, emotional, and cultural needs. Once identified, our team will work to address any barriers to care by ensuring access to the appropriate resources. The SDCC utilizes a holistic approach. We will work with members of your multidisciplinary team, as well as our community partners, to ensure your individual healthcare needs are met.

  • Vaccinations are up to date 
  • Referrals are made for the appropriate testing, therapy, and/or screenings

Lung Cancer Screening

The U.S. Preventive Services Task Force, or USPSTF, recommends annual screening for lung cancer with low-dose computed tomography (LDCT) in adults aged 50 to 80 years with a 20 pack-year smoking history that currently smoke or have quit within the past 15 years.The SDCC will ensure annual screenings are scheduled if applicable.  

Transportation

We recognize that transportation barriers often prevent or delay healthcare for many Delawareans, particularly those living in rural areas. The Southern Delaware COPD Coalition is committed to addressing this need and will provide and/or coordinate transportation to medical and social service appointments.
